The Supportive Housing Worker role is within the Clustered Apartments Program, which provides on-site monitoring and interventions for individuals requiring intensive care and support, including assistance with daily living skills. This program offers low-income subsidized housing to individuals with disabilities, mental illnesses, and/or poverty. The worker will assist with community support services, coordinate mental health services, develop community resources, and function as part of a multi-disciplinary team providing comprehensive assessment and treatment.
