The Supportive Housing Worker will assist with the delivery of community support services for individuals in the Clustered Apartments Program. This role involves coordinating mental health services, developing community resources, and functioning as part of a multi-disciplinary team to provide comprehensive assessment and treatment services to clients. The Clustered Apartments Program offers on-site monitoring and interventions for individuals requiring intensive support and daily living skills training, providing low-income subsidized housing assistance to persons with disabilities, mental illnesses, and/or poverty.
