About The Position

The Supported Housing Coordinator provides leadership, guidance, and support to the Supported Housing team while helping ensure that individuals receiving services have the tools and resources they need to maintain housing, work toward their goals, and live successfully in the community. This position plays an important role in the day-to-day coordination of the program, including staff supervision, training, quality assurance, documentation, program development, and collaboration with community partners. The Coordinator will also maintain a smaller caseload, allowing them to stay connected to the individuals we serve while supporting the team as a whole.

Requirements

  • Minimum of a Bachelor’s degree required.
  • Minimum of two (2) years in the provision of mental health services, including promoting recovery, providing crisis management, and coordinating and collaborating with community resources.

Nice To Haves

  • Background working with individuals with mental health needs in a community-based setting preferred.
  • Supervisory experience preferred.
  • Familiarity with electronic health records and service documentation preferred.
  • Bilingual English/Spanish speaking is a plus!
  • A valid driver’s license and reliable transportation may be required for position-related travel.
  • Occasional lifting of > 15+ pounds.

Responsibilities

  • Supervise Supported Housing staff, provide coaching and guidance, and conduct performance evaluations.
  • Provide training, support, and feedback regarding best practices, agency policies, regulatory requirements, and quality service delivery.
  • Facilitate staff meetings, encourage open communication, and help create a positive and collaborative team environment.
  • Monitor program goals, outcomes, documentation, and reports to ensure services are timely, effective, and meeting expectations.
  • Conduct Quality Assurance (QA) reviews of staff documentation to ensure it is accurate, thorough, and completed within required timeframes in the electronic health record.
  • Maintain a smaller caseload and provide direct support to recipients, including assisting with individualized goals, housing stability, recovery, and community integration.
  • Represent the agency at community meetings, including SPOA and other meetings involving service coordination and community resources.
  • Assist staff and recipients with challenges, conflicts, crises, and barriers that may impact housing stability or successful community living.
  • Work closely with community providers, treatment teams, families/support systems when appropriate, and other service professionals to coordinate effective services.
  • Assist with program development, monitoring, evaluation, and identifying opportunities to improve services and outcomes.
  • Participate in off-hours on-call and emergency response responsibilities as assigned.
  • Work occasional evenings and weekends when needed to ensure the needs of recipients and the program are met.
  • Participate in agency, program, and community meetings as required.
  • Assist with special projects, program initiatives, and program development activities.
  • Provide additional support to the Supported Housing team during periods of increased need.
  • Assist with identifying creative solutions to improve services and strengthen the program.
  • Take on additional assignments as needed.
