Supportive Housing Case Manager
About The Position
Compass Career Solutions, a sister company to Dungarvin, is seeking a Supportive Housing Case Manager to provide case management services and life skill development for homeless individuals and families. This role requires daily in-office work and community outreach, and is not remote. The position involves conducting assessments, developing action plans, providing life skills development, documenting care, collaborating with team members, intervening in eviction risks, connecting clients to services, and guiding them through applications for various social services. The role also includes occasional home visits, enlisting property management support, tracking outcomes, attending meetings, providing transportation to clients, representing Compass at professional events, and utilizing assigned technology.
Requirements
- Resume includes bulleted details of direct housing experience.
- Bachelor's Degree in social services or related field and a minimum of one year experience in the coordination of supportive housing or in the coordination of independent living services in a social service setting OR High School Diploma or GED and two years of experience in the coordination of supportive housing or in the coordination of independent living services in a social service setting.
- Self-starter, excellent time management skills, and proactive problem solver.
- Ability to work a flexible schedule.
- Technology savvy, especially with Microsoft Office Applications, Zoom, PC, and Android devices.
- A valid driver’s license, insurance, and access to a vehicle insured for you as a driver are required.
- Ability to perform CPR/First Aid as needed.
- Ability to lift up to 50 lbs.
- Must be at least 21 years of age.
- Good reading, writing, and communication skills.

Responsibilities
- Conduct assessments and interviews with clients to determine eligibility.
- Develop and update action plans, case notes, and other documents related to a client’s file.
- Provide ongoing life skills development to assist the client in improving independence and quality of life.
- Document care, follow support plans, and collaborate with both internal and external team members.
- Provide specialized and targeted interventions when clients are at risk of eviction.
- Connect clients to medical, mental health, and other supportive counseling services.
- Establish a network of community services and conduct appropriate referrals to clients.
- Guide clients through the process of application, qualification, and re-determination for SSI, SSDI, Medicaid, Medicare, SNAP, and other services as needed.
- Occasionally conduct visits in the client’s home.
- Enlist the support of property management as appropriate for wellness checks.
- Track and report outcomes. Provide data updates as directed.
- Attend and actively participate in scheduled internal and external meetings.
- Travel throughout the area and provide transportation to clients as needed.
- Represent Compass at professional meetings, community events, and to members of the community.
- Ability to use assigned technology as directed in the community (laptop, cell phone, and designated software).
