About The Position

Empower Lives as a Supported Living Program Case Manager. Clinical Supervision toward LCSW licensure available for eligible ASWs. We're seeking an experienced, person-centered professional to oversee Supported Living services for adults with disabilities. Toolworks' Supported Living Program serves the San Francisco, San Mateo County, and East Bay Areas. As a Program Case Manager, you will be at the heart of our mission, ensuring that our clients lead fulfilling lives in the communities of their choice. The Program Manager supervises Community Living Instructors and Leads, coordinates individualized services for a caseload of six to eight clients, monitors service quality and regulatory compliance, and supports clients in achieving greater independence. Your role will be pivotal in providing support, quality assurance, and effective management to enhance the well-being of those we serve. Candidates with an MSW and active California ASW registration are encouraged to apply. Toolworks may provide BBS-qualified clinical supervision toward LCSW licensure for eligible employees. Qualifying experience will depend on assigned duties, the supervision plan, and California BBS requirements; not all hours worked may qualify toward licensure.

Requirements

  • A clean DMV printout, First Aid/CPR certification, and the ability to travel within the designated counties are all essential.

Nice To Haves

  • A Bachelor's degree in Special Education, Rehabilitation Counseling, Social Work, Human Services, or a related field preferred.
  • A Master of Social Work degree and current California Associate Clinical Social Worker registration are highly desirable.
  • At least three years in a role supporting adults with disabilities, with a preference for those with case management experience.
  • Supervisory experience is also valued.
  • Exceptional report writing, strong time management, and a knack for recognizing and resolving challenges.

  • Recruit, train, and mentor your team, ensuring they are equipped and inspired to provide exceptional support.
  • Oversee scheduling, training compliance, and safety procedures, ensuring smooth operations and the highest quality of service.
