About The Position

This position is responsible for the management and supervision of staff in the Supported Living Homes assigned to their caseload. It requires the successful candidate to possess the ability to perform a wide variety of duties. The Coordinator will provide supervision of the Team Leaders and DSP's in the assigned homes and will provide training, mentoring and direction. Participate in the disciplinary actions of the staff supervised, complete annual evaluations and make recommendations for transfers, promotions, etc. The overall goals of this position are to assist the Director in the daily operations of the Supported Living Department, to train and supervise the Supported Living Team Leaders, and to ensure each home is well-run and provides superb living environments for the people in our care.

Requirements

  • High school diploma required.
  • 2+ years of experience in the field required.
  • Competent and efficient computer skills are required.
  • Successful completion of the Medication Administration Course.
  • Valid Driver's License with an F endorsement.
  • Proof of current auto insurance coverage.
  • Personal vehicle must pass quarterly inspection.
  • Must be able and willing to work and meet the demands of a flexible schedule and participate in on-call rotation.
  • Must be able to work various shifts in the individual's homes.
  • Successful completion of a physical examination.
  • Ongoing ability to meet the essential duties of the job.

Nice To Haves

  • Bachelor's Degree preferred.
  • Supervisory experience strongly preferred.

Responsibilities

  • Manage and supervise staff in assigned Supported Living Homes.
  • Provide supervision, training, mentoring, and direction to Team Leaders and DSP's.
  • Participate in disciplinary actions, complete annual evaluations, and make recommendations for staff transfers and promotions.
  • Assist the Director in daily operations of the Supported Living Department.
  • Ensure homes are well-run and provide excellent living environments.
  • Know and keep up with all aspects of assigned homes, staff, and individuals.
  • Spend quality time in homes, read and learn support plans, and ensure individual goals are carried out.
  • Ensure individuals are cared for per policy and regulations.
  • Ensure homes are well-maintained.
  • Complete quarterly Quality Improvement Reports for assigned homes.
  • Train, supervise, and mentor Supported Living Team Leaders.
  • Fill-in for Team Leaders when they are absent, including working open shifts.
  • Conduct unannounced drop-in visits (at least 1 per week), including one true 3rd shift visit per month.
  • Complete, monitor, and follow up with HR and training requirements for staff.
  • Monitor employee orientation process and ensure thorough training.
  • Monitor daily notes for quality content and accurate documentation.
  • Develop and monitor weekly schedules for persons receiving day services.
  • Conduct monthly visits to individuals receiving day services.
  • Monitor site schedules to minimize overtime.
  • Be available to Team Leaders and assist with scheduling.
  • Encourage Team Leaders to develop their own pool of substitute staff.
  • Assist Team Leaders with shopping for food and supplies if needed.
  • Communicate daily with Team Leaders to ensure needs are met and schedules are maintained.
  • Monitor menus and assist Team Leaders with menu development following dietary guidelines.
  • Ensure all mealtime guidelines are followed and be trained as a designated trainer for mealtime instructions.
  • Monitor assigned individuals' health and be aware of medical appointments, potentially attending critical appointments.
  • Be knowledgeable of and monitor individual's medications and medication changes.
  • Obtain consents for Psychotropic Medications and changes timely.
  • Check the computer daily for appointments and alert Team Leaders.
  • Ensure MAR book accompanies individuals to appointments and necessary forms are completed.
  • Ensure Human Rights consents/Psychotropic medication consent forms are signed before the first dose.
  • Attend individual meetings as requested or necessary.
  • Be knowledgeable about all Supported Living individuals.
  • Assist in the development and opening of new homes.
  • Assist Team Leaders in training staff for new homes.
  • Maintain and monitor Cost Plans, ensuring needed services are obtained.
  • Ensure progressive discipline policy and process is utilized.
  • Perform other duties as assigned by the Director and/or Administration.
