About The Position

Unity House of Troy’s Supported Housing Program is looking for a compassionate, dependable, and community-minded professional to join our team as a Supported Housing Case Manager. In this role, you will build meaningful relationships with individuals living with HIV/AIDS and provide the support, encouragement, and connections they need to maintain stable housing and improve their overall quality of life. Requirements

  • Education and/or experience in human services, social services, housing, case management, or a related field preferred.
  • Experience working with individuals experiencing complex health, housing, or social service needs is a plus.
  • Strong communication and interpersonal skills with the ability to build trusting relationships.
  • Excellent organizational and time-management skills.
  • Ability to work independently while also being an effective member of a team.
  • Compassion, patience, professionalism, and a commitment to treating every individual with dignity and respect.
  • Ability to maintain confidentiality and accurate documentation.
  • A genuine desire to make a positive difference in the lives of others.

Responsibilities

  • Build trusting, supportive relationships with program participants through regular and meaningful contact.
  • Provide person-centered, trauma-informed support and counseling to individuals living with HIV/AIDS.
  • Help participants maintain stable housing and address challenges that may affect their housing, health, or overall well-being.
  • Assist participants in identifying goals, needs, and resources that support greater stability and independence.
  • Provide crisis intervention and coordinate emergency services when needed.
  • Connect participants with healthcare providers, community resources, and other supportive services.
  • Collaborate with Unity House staff and outside providers to ensure participants receive coordinated and comprehensive support.
  • Advocate for participants and help them navigate healthcare, housing, and community service systems.
  • Maintain accurate and timely case notes, documentation, and required reports.
  • Promote a welcoming environment grounded in inclusion, dignity, respect, and belonging.
  • Perform other duties as assigned.
