About The Position

This is a part-time, benefit-eligible position in Shenandoah, Iowa. The hours are Thursday and Friday from 2:00 PM to 11:00 PM, and every other weekend from 9:00 AM to 9:00 PM. The Supported Community Living Specialist (SCLS) is directly responsible to the Team Leader to which they are assigned and the Director of Residential Services. This is a para-professional level position. The SCLS will teach, train, and assist individuals in developing the skills necessary to be as independent as possible when performing activities of daily living. This includes coordinating and training individuals to take care of their own health and medical needs, developing or maintaining skills for better community participation, assisting individuals in becoming independent in accessing community services, and assisting with participation in community activities. The role also involves assisting individuals in advocacy procedures, preparing individualized diet plans, writing Individualized Intervention Plans, providing emergency/safety skills training, training in managing personal business and property, budgeting, and providing transportation for appointments and other needs. Additionally, the SCLS will orient new individuals, assess skills, provide input for progress reports and Person-Centered Plans (PCP), supervise goals and data collection, supervise finances, provide a supportive and therapeutic atmosphere, supervise care and use of personal property, train new staff, follow the assigned work schedule, ensure individuals receiving twenty-four-hour care are never left unsupervised, complete all records and documentation, attend staff meetings and in-service training, ensure residence meets cleanliness, safety, and health standards, stay informed of programming and administrative information, maintain confidentiality, document behaviors, and handle other assigned duties. The role also includes monitoring physician-ordered diets and assisting individuals with activities of daily living such as meal preparation, laundry, housekeeping, toileting, and bathing, and ensuring clients are safe from environmental hazards.

Requirements

  • Individuals must not have or be a carrier of serious infectious or communicable disease that cannot be eliminated or reduced by reasonable accommodation (e.g., tuberculosis).
  • 18 years of age.
  • High school diploma or GED.
  • Exercise sound judgment when the situation requires immediate decision-making.
  • Obtain and maintain a valid Iowa Chauffeur’s license.
  • Maintain acceptable driving record as determined by agency insurance carrier.
  • Successful completion of pre-employment physical, TB Tine, and drug screenings.
  • Complete 10-hour med-management class.
  • Willing to work with individuals who have a behavioral disorder, which may result in aggressive outbursts.
  • Willing to clean biological fluids/waste, such as blood, feces, urine, or vomit.
  • Ability and desire to work with other employees as a team.
  • Good verbal and written communication skills.
  • Self-motivated.
  • Must not have been legally convicted of any type of assault, abuse, or bodily injury.
  • All agency staff are required to participate and cooperate with agency investigations.

Nice To Haves

  • Previous similar job experience in teaching skills of cooking, cleaning, and personal hygiene.

Responsibilities

  • Teach, train, and assist individuals in developing skills for independence in activities of daily living.
  • Coordinate health and medical needs and train individuals to manage their own health.
  • Develop or maintain skills for better community participation and assist individuals in accessing community services.
  • Assist individuals with participation in at least one community activity each week if shift is during day/evening/weekend hours.

  • Write Individualized Intervention Plans for each individual served.
  • Provide emergency/safety skills training.
  • Provide training in skills necessary to manage personal business and property.
  • Provide training in budgeting.
  • Provide transportation for doctor’s appointments, church, etc.
  • Provide training in daily living skills.
  • Orient new individuals in their home, assess their skills, and provide input to progress reports and Person-Centered Plans (PCP).
  • Supervise the individual’s goals and complete data collection.
  • Supervise, as necessary, the individual’s finances to give guidance in budgeting.
  • Provide a supportive and therapeutic atmosphere.
  • Supervise the individual’s care and use of personal property and assist with purchases.
  • Train new staff relative to duties, work schedules, agency policies, and state and/or federal regulatory policies and procedures.
  • Follow the assigned work schedule.
  • Never leave individuals receiving twenty-four-hour care unsupervised.
  • Complete all records and documentation using the proper forms.
  • Attend staff meetings and scheduled in-service training sessions.
  • Ensure that the individual’s residence meets cleanliness, safety, and health standards.
  • Keep informed of all programming and administrative information, changes, and additions.
  • Keep all written and verbal information on each individual strictly confidential.
  • Complete documentation of behaviors or provide information for writing staff intervention plans.
  • Handle any other duties as assigned.
  • Monitor and document an individual’s physician ordered diet as needed.
  • Assist individuals receiving services with their activities of daily living, such as meal preparation, laundry, housekeeping, toileting, and bathing.
  • Ensure the clients are safe from all environmental hazards.
  • Provide quality care.
  • Administer medication and maintain accurate medication documentation, as needed, after being trained.
  • Exercise sound judgment when the situation requires immediate decision-making.
  • Comply with state and federal regulations regarding HIPAA.
  • Ensure housekeeping standards are maintained.
  • Work with individuals who have behavioral problems, which may include verbal and physical aggression.
  • Remain awake during overnight shifts or be available for emergencies as per shift requirements.
  • Complete written and electronic documentation and record keeping according to agency requirements.
  • Work independently and ensure the welfare and safety of persons served in a residential setting.
