This is a full-time position with hours ranging from 34-40 per week. The Supported Community Living Specialist (SCLS) is directly responsible to the Team Leader to which they are assigned and the Director of Residential Services. This is a para-professional level position. The SCLS will teach, train, and assist individuals in developing the skills necessary to be as independent as possible when performing activities of daily living. This includes coordinating health and medical needs, training individuals to manage their own health, developing or maintaining skills for community participation, assisting individuals in accessing community services, and assisting with participation in community activities. The role also involves assisting individuals in advocacy, preparing individualized diet plans, writing Individualized Intervention Plans, providing emergency/safety skills training, training in managing personal business and property, budgeting, and providing transportation for appointments. The SCLS will orient new individuals, assess skills, and provide input for progress reports and Person-Centered Plans. They will supervise individual goals and data collection, and as necessary, supervise individual finances to provide guidance in budgeting. The role requires providing a supportive and therapeutic atmosphere, supervising the care and use of personal property, and assisting with purchases. Additionally, the SCLS may train new staff, follow assigned work schedules, ensure individuals receiving twenty-four-hour care are never left unsupervised, and complete all records and documentation according to procedures. Attendance at staff meetings and in-service training is required. The SCLS must ensure residences meet cleanliness, safety, and health standards, stay informed of programming and administrative information, and maintain strict confidentiality of all individual information. They will document behaviors or provide information for intervention plans and handle other assigned duties. The role involves monitoring and documenting physician-ordered diets and assisting individuals with activities of daily living such as meal preparation, laundry, housekeeping, toileting, and bathing, while ensuring clients are safe from environmental hazards.
