About The Position

This is a part-time, benefit-eligible position averaging 30 hours per week, located in Red Oak, Iowa. The schedule includes Mondays and Tuesdays from 3:00 PM to 11:00 PM, and every other weekend from 9:00 AM to 11:00 PM. The Supported Community Living Specialist (SCLS) reports directly to their assigned Team Leader and the Director of Residential Services. This role is considered a para-professional level position. The SCLS is responsible for teaching, training, and assisting individuals in developing skills for independence in daily living activities, coordinating health and medical needs, facilitating community participation, assisting with accessing community services, and supporting individuals in advocacy and personal business management. Responsibilities also include writing Individualized Intervention Plans, providing emergency/safety skills training, budgeting assistance, transportation, and training in daily living skills. The role involves orienting new individuals, assessing skills, completing progress reports and Person-Centered Plans, supervising goals and data collection, and managing individual finances. A supportive and therapeutic atmosphere is to be maintained, along with supervision of personal property use and purchases. New staff may be trained on duties, schedules, and agency policies. Adherence to the assigned work schedule and ensuring individuals receiving twenty-four-hour care are never left unsupervised are critical. All records and documentation must be completed accurately, and staff are expected to attend meetings and in-service training. Maintaining cleanliness, safety, and health standards in residences, staying informed of programming and administrative changes, and strictly maintaining confidentiality of individual information are also key duties. The role includes documenting behaviors, handling assigned duties, monitoring physician-ordered diets, and assisting individuals with activities of daily living such as meal preparation, laundry, housekeeping, toileting, and bathing. Ensuring client safety from environmental hazards is paramount.

Requirements

  • Individuals must not have or be a carrier of serious infectious or communicable disease, such as tuberculosis, which cannot be eliminated or reduced by reasonable accommodation.
  • 18 years of age.
  • High school diploma or GED.
  • Exercise sound judgment when the situation requires immediate decision-making.
  • Obtain and maintain a valid Iowa Chauffeur’s license.
  • Maintain acceptable driving record as determined by agency insurance carrier.
  • Successful completion of pre-employment physical, TB Tine, and drug screenings prior to employment.
  • Complete 10-hour med-management class.
  • Must be willing to work with individuals who have a behavioral disorder, which may result in the display of aggressive outbursts of a physical nature.
  • Must be willing to clean any biological fluids/waste, such as blood, feces, urine, or vomit.
  • Must have the ability and desire to work with other employees as a team.
  • Must have good verbal and written communication skills.
  • Must be self-motivated.
  • Must not have been legally convicted of any type of assault, abuse, or bodily injury.
  • All agency staff are required to participate and cooperate with agency investigations.
  • Must be able to travel from place to place, up to 70 miles.
  • Must be able to enter and exit resident homes that may or may not be accessible to everyone.
  • Must be able to walk up and down stairs with hands full to perform laundry duties, etc.
  • Must be able to work a full scheduled shift as assigned.
  • Must be able to carry necessary supplies up to 50 pounds to perform job duties.
  • Must be able to provide physical care necessary to residents when performing direct care duties.
  • Ability to physically assist an individual to regain his balance or to transfer from a sitting, standing or prone position.
  • Demonstrate ability to perform routine household tasks, such as vacuuming, mopping, use of cleaning agents, meal preparation, kitchen clean up, and clothing care.
  • Must be able to assist an individual in a wheelchair to transfer to another chair, bed, toilet, or bath.
  • Must be able to assist someone in the shower or bath.
  • Must be able to make multiple trips from car to house carrying purchases such as groceries, etc.
  • Must be able to perform snow removal as much as necessary for individual and staff safety.

Nice To Haves

  • Previous similar job experience in teaching skills of cooking, cleaning, and personal hygiene.

Responsibilities

  • Teach, train, and assist individuals in developing skills for independent daily living activities.
  • Coordinate health and medical needs and train individuals to manage their own health.
  • Develop or maintain skills for better community participation and assist individuals in accessing community services.
  • Assist individuals with participation in at least one community activity each week if shift is during day/evening/weekend hours.
  • Assist individuals in advocacy procedures to ensure their rights and meet their needs, interests, and choices.
  • Assist in preparing an individualized diet plan to meet nutritional needs as needed.
  • Write Individualized Intervention Plans for each individual served.
  • Provide emergency/safety skills training.
  • Provide training in skills necessary to manage personal business and property, including budgeting.
  • Provide transportation for appointments, church, etc.
  • Provide training in daily living skills.
  • Orient new individuals in their home, assess their skills, and provide input to progress reports and Person-Centered Plans (PCP).
  • Supervise individual goals and complete data collection.
  • Supervise, as necessary, individual finances to provide guidance in budgeting.
  • Provide a supportive and therapeutic atmosphere.
  • Supervise the individual’s care and use of personal property and assist with purchases.
  • Train new staff relative to duties, work schedules, agency policies, and state and/or federal regulatory policies and procedures.
  • Follow the assigned work schedule.
  • Never leave individuals receiving twenty-four-hour care unsupervised.
  • Complete all records and documentation using the proper forms as they relate to established procedures.
  • Attend staff meetings and scheduled in-service training sessions.
  • Ensure that the individual’s residence meets cleanliness, safety, and health standards.
  • Stay informed of all programming and administrative information, changes, and additions by checking email, the daily logbook, or other messages each shift.
  • Keep all written and verbal information on each individual strictly confidential.
  • Complete documentation of behaviors or provide information for writing staff intervention plans.
  • Handle any other duties as assigned by the Team Leader, Lead Staff, and Director of Residential Services.
  • Monitor and document an individual’s physician ordered diet as needed.
  • Assist individuals receiving services with their activities of daily living, such as meal preparation, laundry, housekeeping, toileting, and bathing.
  • Ensure the clients are safe from all environmental hazards.
  • Provide quality care.
  • Administer medication and maintain accurate medication documentation, as needed, after being trained.
  • Exercise sound judgment when the situation requires immediate decision-making.
  • Complete written and electronic documentation and record keeping according to agency requirements.
  • Work independently and ensure the welfare and safety of persons served in a residential setting.
  • Comply with state and federal regulations regarding HIPAA.
  • Participate and cooperate with agency investigations.
