About The Position

This position is responsible for developing and maintaining relationships with program participants, as well as finding and utilizing community resources that may benefit them. The role involves identifying the treatment and service needs of each participant, monitoring and evaluating the progress of their treatment plan, and assisting as appropriate. This position also helps to manage expenses, bill hours, and write case notes. They assist the Supported Community Living Specialist with service plan developments and progress reports. Travel between participant locations is required.

Requirements

A high school diploma or GED required
Six months experience in training and/or rehabilitation required.
Reliable vehicle, valid Iowa driver’s license, proof of auto-insurance, and successful completion of a motor vehicle record screening, dependent and adult child abuse, and criminal background check

Post-secondary education may be substituted for experience requirement.

Responsibilities

Develop and maintain effective relationships with clients.
Identify and maintain current information about community resources that may benefit program participants.
Identify treatment and service needs of each individual served.
Monitor and evaluate progress of the treatment plan, assisting and facilitating as appropriate.
Monitor and control expenses and ensure efficiency in rehabilitation services, including billable hours and client funding.
Write quality case notes and assist SCL Specialist with ISP Development and Progress Reports.
Provide transportation to participants as needed.
