About The Position

This position is responsible for admission, assessments and orientation, carrying a caseload, and group facilitation. This position also includes some responsibility in assisting the Reintegration Manager with oversight of day-to-day responsibilities. The role involves managing service programming, assisting with staff development, overseeing case records, facilitating groups, documenting progress, coordinating services with external providers, and managing educational/vocational skill development. The coordinator also assists with resident admissions and orientations, performs screenings, responds to crises, and handles data input. Additionally, the position requires adherence to confidentiality regulations (42 CFR, Part 2, HIPAA, Medicaid/HUD), participation in staff meetings and committees, and the integration of Trauma-Informed Care principles. On-call responsibilities and 24-hour coverage may be required.

Requirements

  • Must possess and maintain an active Qualified Health Professional (QHP) CASAC-Advanced Status.
  • Bachelor’s Degree in Human Services.
  • At least 6 months’ experience in the substance use, mental health, or human services fields.
  • Completion of OASAS Clinical Foundations Trainings 1 & 2.
  • Valid New York State Driver’s License.

Responsibilities

  • Overall management of service programming in conjunction with the Reintegration Manager.
  • Carrying a small caseload.
  • Assisting the Reintegration Manager with the development of other program staff members.
  • Overseeing the maintenance of the program’s individual case records in compliance with agency policy and licensing authority regulations.
  • Developing and assisting other staff members with service planning and discharge planning.
  • Facilitating weekly didactic and process groups as needed.
  • Documenting individual progress in accordance with agency policy and procedure.
  • Meeting with residents regularly to monitor progress and revising individual service plans as needed.
  • Making referrals, coordinating services, and maintaining regular ongoing consultations with other providers.
  • Coordinating educational and/or vocational skill development.
  • Organizing and maintaining case records.
  • Assisting with the admission process and orientation of new residents as needed.
  • Providing breathalyzer and toxicology screening as needed.
  • Responding to any crises and emergencies.
  • Completing data input into the agency database.
  • Responding in a professional and responsible manner to incidents involving contagious diseases.
  • Assisting with the program’s medication policies and procedures.
  • Being available for on-call responsibilities.
  • Being available for 24-hour coverage when needed.
  • Participating in weekly staff meetings and case conferences.
  • Serving on agency committees as requested.
  • Carrying out additional duties essential to the above-stated responsibilities as assigned.
  • Being mindful of and integrating Trauma-Informed Care and its five principles in all duties and interactions.
  • Maintaining compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others.
  • Regular attendance at work location.
