The Support Services Coordinator is responsible for supervising and supporting daily needs of the Home Health and Lifeline departments, as well as Community Care needs when appropriate. This position does not directly manage caregivers. The Support Services Coordinator role is a temporary, part-time position expected to last 90 days. The position is part of the St. Charles Health System Community Care department, which supports the health system mission by providing innovative care to achieve better health, better care, and better value. Community Care helps to bridge any gaps in the needs of patients as they navigate through the continuum of care, and throughout the life cycle, providing care and supportive services to the communities we serve. The services encourage a focus on providing care for post-acute conditions and outpatient care within the realm of Home Health, Hospice, and Palliative Care, while developing strong relationships, service line collaboration and referrals of higher acuity conditions. Care is provided to patients and families as part of an integrated system of settings, services, and care levels with the goal of helping our patients live as independently as possible with a higher quality of life, even and especially during advancing illness and end-of-life care.
