About The Position

The Support Services Associate/Community Health Worker (CHW) serves a dual role by providing administrative support for health department programs while assisting with community outreach, education, and program coordination. This position helps ensure the efficient operation of clinic and public health services through accurate recordkeeping, customer service, and collaboration with community partners. The Support Services Associate/CHW serves as a resource for patients, community members, and partner organizations while supporting health initiatives that improve the well-being of the community.

Responsibilities

  • Coordinate assigned community health programs, projects, coalition meetings, and related activities.
  • Maintain collaborative relationships with community partners.
  • Represent the health department on interagency councils, committees, and community meetings as assigned.
  • Develop and implement program policies and procedures to ensure compliance, accountability, and program effectiveness.
  • Prepare statistical reports on program performance, needs, services, information and demographic data.
  • Provide information and interpretation of the assigned program project to all staff and general public.
  • Other Health Education duties as assigned.
  • Route and direct the public to the right and correct information.
  • Prepare routine letters, labels and other appropriate material.
  • Maintain medical records and files.
  • Collect and record fees obtained to ensure completeness and accuracy.
  • Enter information from the Patient Encounter Forms (PEF) Outcome or Assessment Information Set (OASIS).
  • Generate and issue Woman Infant and Children (WIC) vouchers.
  • Request and review routine reports.
  • Gather information from patients and insurance.
  • Request and reviews relevant reports for completeness.
  • Complete forms containing all pertinent information for processing.
