About The Position

The Support Counselor (SC) plays a crucial role in assisting children, youth, their families, and adults by providing support, education, and advocacy to help them successfully navigate various systems. The SC ensures all team members are actively involved in the planning and linkage process. The primary focus of this role is to deliver targeted case management and rehabilitative services aimed at stabilizing individuals and reducing involuntary hospitalizations and placement disruptions. The program, TEAMMATES, offers Community-Based and Family Centered services, including Wraparound, Intensive Field Capable Clinical Services (IFCCS), IFCCS Outreach Triage Team, and Child Outreach Triage Team services, covering a full spectrum of treatment and support. Services are delivered countywide within 24 hours of referral receipt and include 24-hour Crisis Intervention for children and adolescents with severe emotional disturbances. The program is client and family-driven, with services mutually agreed upon by providers and families, and operates under contracts with DMH, Child Protected Services, and Probation. Requirements

  • SC I: High School diploma or equivalent (e.g. GED, HiSET, etc.) required.
  • SC II: Associates degree in a Mental Health, Human Services or closely related field.
  • SC III: Bachelor’s degree in Mental Health, Human Services, or closely related field.
  • SC I: Three (3) years experience with severely emotionally disturbed children and families.
  • SC II: Two (2) years of experience with severely emotionally disturbed children and families.
  • SC III: One (1) year experience working with severely emotionally disturbed children and/or families.
  • Valid California Driver’s License.
  • Spanish Required.

Responsibilities

  • Take a lead role and responsibility for working with children, youth, their families, and adults to provide support, education, and advocacy to successfully navigate systems.
  • Ensure all team members are active participants in the planning and linkage process.
  • Focus on a broad range of targeted case management and rehabilitative services meant to provide stabilization and successful reduction of involuntary hospitalization and placement disruption.
