Requirements

  • Strong interpersonal skills.
  • Proficient use of relevant software and tools.
  • Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling.
  • One (1) year of experience in support coordination services with the assigned population.
  • Valid Virginia Driver's License.
  • Deep understanding of advocacy, crisis intervention, and community resources.
  • Must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.

Responsibilities

  • Assist clients with their basic physical, financial, social, and personal needs.
  • Develop person-centered plans tailored to each individual.
  • Link clients with service providers.
  • Monitor progress.
  • Advocate for clients' needs both within and outside the system.
  • Guide families through the complex service landscape.
  • Foster informed choices about available supports.
  • Enhance community integration.
  • Address medical, educational, vocational, and recreational needs.
  • Ensure clients achieve desired outcomes aligned with discharge protocols.
