This role involves assisting clients by assessing their needs for support services and facilitating access to various programs and services, including vocational, medical, psychological, and in-home support. The Support Coordinator determines eligibility and secures Medicaid and DDD-funded services, considering all available resources. They develop Individualized Service Plans (ISPs) using a person-centered approach, involving the participant and their team. The role includes facilitating ISP implementation, monitoring service delivery, conducting in-home visits, and documenting findings. Experienced coordinators may also assist with training new staff, provide backup to supervisors, and attend promotional events. The position requires regular travel throughout specific counties in New Jersey and the ability to work outside regular business hours and be on-call as needed.
