This role involves assisting clients by assessing their needs for support services and facilitating access to various programs and services, including vocational, medical, psychological, and in-home support. The Support Coordinator determines eligibility, secures funding (Medicaid and DDD-funded services), and ensures all available resources are considered. A key responsibility is developing Individualized Service Plans (ISPs) using a person-centered approach, with input from the client and their team. The role also includes facilitating the implementation of ISPs, monitoring service delivery, conducting in-home/on-site visits, and documenting findings. Experienced coordinators may be assigned Assistant Supervisor duties, including training new coordinators, providing caseload support, and acting as a backup to the supervisor. This may involve attending promotional events and providing on-call coverage.
