About The Position

Boone County Family Resources is a rapidly growing community-based agency committed to supporting Boone County residents with developmental disabilities and their families to THRIVE in our community, CONNECT with others and ACHIEVE their personal goals. Support Coordinators work collaboratively with individuals, families, and other professional team members to develop and implement individual support plans. Support Coordinators connect individuals and families with resources and services in the community and then monitor the effectiveness of those supports in helping individuals meet their needs and achieve personal goals.

Requirements

• At least 21 years old
• Bachelor's degree
• One year of related experience working with individuals with developmental disabilities
• Valid driver’s license
• Access to an insured and properly maintained vehicle for transportation
• Operate a vehicle in a safe and prudent manner to comply with all applicable laws

Responsibilities

• Develop and implement individual support plans.
• Connect individuals and families with resources and services in the community.
• Monitor the effectiveness of supports in helping individuals meet their needs and achieve personal goals.
