About The Position

Experienced Support Coordinator (Part-Time) for Burlington County. We seek a self-motivated and compassionate individual with experience working with adults with intellectual and developmental disabilities. This hybrid position requires field visits; candidates must be flexible to travel within these counties. The state of New Jersey requires Covid vaccination for this position. Must pass criminal background and CARI check. We are New Jersey’s #1 Support Coordination Agency. NLC Support is a Support Coordinating Agency for adults and children with developmental disabilities. Our mission statement is to be here for them, from start to finish. We are NJ’s #1 Support Agency. Period. Choosing a support coordination agency is a new experience for a majority of people. Support Coordinators here at NLC make it possible for families to match the person’s wants and needs with skills and expertise. Since people are not aware of what is available in their community, we help find providers and services that are in their area.

Requirements

  • One year of proven experience working with adults with developmental disabilities
  • Candidates must be able to take the initiative, work independently, make executive decisions when necessary, and meet deadlines
  • Excellent written and verbal communication
  • Reliable car
  • Flexibility to attend meetings when needed during the day
  • Must reside in the State of New Jersey
  • Must pass criminal background and CARI check
  • A bachelor’s degree in psychology, sociology, social work, or related field
  • Covid vaccination

Responsibilities

  • Assessing service needs
  • Developing the New Jersey Individualized Service Plan (NJISP) for access to services
  • Liaison with clients, families, programs, and service providers
  • Establish achievable goals, outcomes, and service eligibility
  • Development of a Person-Centered Planning Tool (PCPT) is a mandatory discovery tool used to guide the person-centered planning process and assist in developing the NJISP
  • Ongoing monthly monitoring of clients
  • Coordinate and facilitate team meetings off-site at either Residential and/or Day Programs to identify service needs
