The Permanent Supportive Housing (PSH) Program offers a long-term, affordable housing option combined with flexible, voluntary support services to help high-needs individuals experiencing homelessness maintain stable housing and live independently. Delivered through partnerships among multiple agencies, the program assists residents of the PSH building who face significant challenges such as substance use, mental health issues, and other complexities. It focuses on helping them achieve housing stability and improve their quality of life by providing access to housing, practical assistance, counseling, advocacy, landlord mediation, and eviction prevention. As a member of the Allied Health Team, the PSH Support Coordinator works with vulnerable individuals who are residents at the permanent supportive housing buildings at 10 Shelldale, as well as Grace Gardens, and the transitional housing program at 65 Delhi Street, who are facing multiple and complex barriers to accessing healthcare through system navigation and direct, one-to-one support. This position works at multiple sites in the community.
