About The Position

Support Coordinator - Adult Developmental Disabilities Annual Salary: $49,612 Work schedule: Monday - Friday 8:30 am - 5:00 pm Are you passionate about supporting individuals with developmental disabilities? Requirements

Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling.
One (1) year of experience providing case management or support coordination services to adults living with a developmental disability.
Valid Driver's license for transporting clients as necessary.
In-depth knowledge of intellectual and developmental disabilities (IDD), autism, and other developmental, physical, emotional, and sensory disorders.

Responsibilities

Develop personalized care plans and connect individuals living with developmental disabilities to essential services and supports.
Advocacy, counseling, crisis intervention, and daily living support to promote independence and inclusion in the community.
Support these individuals' fundamental physical, financial, social, and personal requirements.
Facilitate connections with service providers.
Provide ongoing monitoring, advocacy, crisis intervention, counseling, and daily living support to empower individuals living with developmental disabilities to lead fulfilling lives.
