About The Position

The Hampton-Newport News Community Services Board is seeking a dedicated Support Coordinator to join their team. This role involves developing personalized care plans and connecting individuals with developmental disabilities to essential services and supports. The Support Coordinator will provide advocacy, counseling, crisis intervention, and daily living support to promote independence and community inclusion. Responsibilities include supporting individuals' fundamental physical, financial, social, and personal requirements, facilitating connections with service providers, ongoing monitoring, and empowering individuals to lead fulfilling lives.

Requirements

  • Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling.
  • One (1) year of experience providing case management or support coordination services to adults living with a developmental disability.
  • Valid Driver's license for transporting clients as necessary.
  • In-depth knowledge of intellectual and developmental disabilities (IDD), autism, and other developmental, physical, emotional, and sensory disorders.
