About The Position

The Supervisor provides clinical, administrative, and programmatic oversight for Community Connections’ BMS Foundation-funded ReEntry Program: A Reentry Diversion Initiative. This two-year demonstration program aims to strengthen behavioral healthcare continuity for adults with serious mental illness and co-occurring substance use disorders transitioning from incarceration to community living. Its goals include improving recovery outcomes, reducing criminal justice involvement, and establishing a sustainable model of community-based reentry services. The Supervisor implements and maintains fidelity to the PTR service model, including pre-release engagement, individualized transition planning, intensive support during the first 30 days after release, and services throughout the six-month post-release intervention period. The role supervises two Case Managers, maintains a limited direct-service caseload, oversees program operations, and ensures trauma-informed, recovery-oriented, culturally responsive services. Working with the DC Department of Corrections, Department of Behavioral Health, Unity Health Care, Chestnut Health Systems, and other community partners, the Supervisor coordinates service delivery and cross-system communication and supports timely access to behavioral healthcare, housing, benefits, and recovery supports. The Supervisor monitors enrollment, outcomes, model fidelity, data quality, and grant deliverables; supports evaluation and continuous quality improvement; and contributes to efforts to establish sustainable financing and future program expansion. The position is contingent upon continuation of grant funding.
