The Supervisor provides clinical, administrative, and programmatic oversight for the Community Connections' BMS Foundation-funded Pathways to Recovery (PTR): A Reentry Diversion Initiative. This two-year demonstration program is designed to strengthen continuity of behavioral healthcare for adults with serious mental illness and co-occurring substance use disorders transitioning from incarceration to community living, with the goals of improving recovery outcomes, reducing criminal justice involvement, and establishing a sustainable model of community-based reentry services. The Supervisor is responsible for implementing and maintaining fidelity to the PTR service model, including pre-release engagement, individualized transition planning, intensive support during the critical first 30 days following release, and continued services throughout the six-month post-release intervention period. The position supervises two Case Managers, maintains a limited direct-service caseload, oversees program operations, and ensures high-quality, trauma-informed, recovery-oriented, and culturally responsive services. Working collaboratively with the DC Department of Corrections, Department of Behavioral Health, Unity Health Care, Chestnut Health Systems, and other community partners, the Supervisor ensures coordinated service delivery, effective cross-system communication, and timely access to behavioral healthcare, housing, benefits, and recovery supports. The Supervisor is also responsible for monitoring participant enrollment, program outcomes, implementation of fidelity, data quality, and grant deliverables; supporting evaluation and continuous quality improvement; and contributing to organizational efforts to establish sustainable financing and opportunities for future program expansion. This position is contingent upon the continuation of grant funding.
