This position has been approved to be Full Office. If telework is approved, you are required to work within the State of California only and if Partial Telework, also come in to the Main Office in Orange, CA, at least two (2) days per week minimum. The Supervisor Social Work for the Program for All-Inclusive Care for the Elderly (PACE) will be responsible for the behavioral health services and direct social work care management services, such as participant screening, case management, counseling and referral to the participants of CalOptima Health PACE. You will work in collaboration with the Manager PACE Center and the Interdisciplinary Team (IDT). Additionally, you will be responsible for the coordination of home care services, such as assessing needs, processing authorizations and oversight of care rendered. Together, we are building a stronger, more equitable health system.

Requirements

  • Master’s degree in social work from an accredited college or university PLUS 3 years of social work experience in a health-related field required; an equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.
  • 1 year of experience working with the frail or elderly population required; relevant experience must be within the last 3 years.
  • 1 year of progressive leadership experience required.
  • Have access to means of transportation for work away from the primary office approximately 15% of the time required.
  • CPR and First-Aid Certification required or must be obtained within six (6) months of date of hire.

Responsibilities

  • Participates in a mission driven culture of high-quality performance, with a member focus on customer service, consistency, dignity and accountability.
  • Assists the team in carrying out department responsibilities and collaborates with others to support short- and long-term goals/priorities for the department.
  • Provides oversight of authorization services completed by the Social Workers.
  • Provides supervision, clinical direction and guidance to the social work team and to associate social workers working towards licensure.
  • Leads social work weekly meetings to discuss high risk cases and provides clinical direction and support.
  • Assists with hiring, develops, trains and evaluates the performance of Social Workers to ensure high quality social work services and standards are met.
  • Reviews and oversees documentation by Social Workers to ensure compliance with industry and departmental standards.
  • Works with PACE administration and direct care staff to develop and revise PACE policies and procedures related to the Social Work department.
  • Participates in the IDT initial assessments, care planning and periodic reassessments (minimally every six months) of the participant's case in accordance with industry and departmental standards.
  • Maintains and builds relationships with appropriate community agencies or facilities to refer participants and families.
  • Provides oversight to ensure that social work services are implemented as identified in the plan of care and in a manner consistent with regulations and quality assurance measures.
  • Participates in program and policy development regarding social work and demonstrates active participation in quality improvement processes.
  • Oversees behavioral health services rendered by social work team as well as outside behavioral health contractors.
  • Develops and leads group counseling and caregiver support group activities.
  • Provides individual and family counseling for participants as part of the behavioral health services team as needed.
  • Refers participants and families to appropriate community agencies or facilities, serves as a liaison with such organizations and advocates for participants.
  • Assists in the completion of advance directives, mortuary arrangements, funeral planning and/or facility placement.
  • Assesses suicide risk, cognitive impairment, child, elder and/or dependent adult abuse and takes appropriate action.
  • May be required to perform home visits to private homes of PACE participants; occasional travel in Orange County.
  • Supports the Social Work department with case coverage needs.
  • Maintains current knowledge and expertise on industry standards for written case management records by continuing education through academic studies, seminars and workshops.
  • Completes other projects and duties as assigned.
