As Supervisor of Clinical Assessment – Care Delivery, you’ll oversee the quality and timely completion of UAS-NY assessments, support clinical assessment nurses and reviewers, and serve as a resource to colleagues across our PACE program.

Requirements

  • Active New York State Registered Nurse license or Physician Assistant license or certification authorizing practice in New York.
  • At least 3 years of experience in MLTC, LTHHCP, MAP, PACE, or another managed care program.
  • Experience in care or case management, disease management, population health, utilization review, quality assurance, or discharge planning.
  • Proficiency using and analyzing UAS-NY; familiarity with clinical management systems and customer relationship management platforms.
  • Strong clinical judgment, staff leadership, communication, documentation, Microsoft Office, and report-writing skills.
  • Ability to work independently, manage deadlines, and travel across Service area for training and field supervision.
  • Experience working with a frail or elderly population, or completion of appropriate PACE-provided training upon hire if otherwise qualified.

Nice To Haves

  • Five years of experience in MLTC, LTHHCP, MAP, PACE, or another managed care program.
  • Experience in managed care, home care, or community health settings.
  • Bilingual language skills.

Responsibilities

  • Lead and develop clinical assessment nurses and reviewers, including training, competency evaluation, quality review, and onboarding.
  • Oversee timely, accurate in-home UAS-NY assessments and reassessments, clinical documentation, and compliance with CMS, NYSDOH, and PACE requirements.
  • Partner with Care Delivery and the interdisciplinary team to communicate clinical findings, support enrollment transitions, and address participant needs after enrollment.
  • Oversee the review of personal care hour authorizations to align services with assessed needs and applicable requirements.
  • Improve team workflows, productivity, and assessment quality; prepare required reports and support quality improvement initiatives.
  • Manage day-to-day relationships with contracted assessment agencies, including staffing coordination, clinical support, and invoice review.
