This position works with nursing team to coordinate care for new patients. Monitors the ongoing home health nurse/patient relationship and patient progress. Directly involved in hiring, coaching, and counseling new field staff, provides orientation and educational training. The position will include establishing and maintaining positive relationships with patients, home health agencies and referral sources. Interacts with the intake department, clinical pharmacists, sales team, administrative departments, and outside agencies. Works with branch leadership to manage performance improvement program of the agency such as chart audits, formulation of Plan of Care (POT/485) and assessing staff’s clinical documentation technique, compliance with documentation guidelines and adherence to nursing standards.
