About The Position

Bayshore HealthCare is seeking an RPN/LPN to join our team in the role of Home Support Supervisor. This role is accountable to implement effective management practices that promote paraprofessionals’ full scope of clinical practice to achieve excellence in the delivery of care and service to clients. The HSS will ensure compliance with internal and external standards of practice and Provincial regulatory requirements. The HSS manages paraprofessional employees and identifies and coordinates the required clinical competencies and in-service activities to deliver high quality client/patient and family-centered care.

Requirements

  • Graduation from an accredited Licensed/Registered Practical Nursing Program
  • Current registration with the provincial regulatory body.
  • At least five years of recent experience with progressive responsibilities, as a Home Support Supervisor, preferably in a community setting
  • Strong track record in leading the development and evaluation of training programs
  • Demonstrated ability to handle all aspects of human resources and interpret company policies and procedures
  • Proven computer technology skills e.g. Microsoft applications, and ability to use virtual meeting forums.
  • Solid understanding of the principles, practices, and methods of community nursing and service delivery
  • Exceptional interpersonal skills and ability to handle difficult situations objectively and consistently
  • Ability to work independently and effectively lead a high performing team
  • Demonstrated team-building skills
  • Excellent writing, reporting and English communication skills
  • Valid driver’s license and auto insurance
  • Evidence of current CPR certification
  • Clear criminal background check including vulnerable sector screening

Nice To Haves

  • Additional training and/or certifications such as CNA certifications, Learning Essentials for Advanced Palliative Care (LEAP) or dementia training would be an asset to this role
  • Prior experience in business development preferred.
  • French language proficiency considered a strong asset

Responsibilities

  • Lead and participate in continuous quality improvement activities aligned with the company’s Quality Management System.
  • Ensure all clients receive holistic assessments and ongoing reassessments to promote positive client and clinical outcomes.
  • Develop inter-professional, client-centered, goal-oriented care plans with clients and families to ensure high-quality, safe care and positive health outcomes.
  • Manage care and services by visiting clients as needed, ensuring satisfaction, safety, and positive outcomes.
  • Supervise and support paraprofessional employees, ensuring compliance with federal and provincial standards and company policies.
  • Escalate concerns, risks, and clinical practice issues to appropriate regulated health professionals.
  • Promote and improve team engagement and client experiences through various methods.
  • Provide constructive, timely, and ongoing feedback and performance management.
  • Facilitate a learning environment by translating evidence-based knowledge into education to optimize patient outcomes and experiences.
  • Attend and participate in internal and external education offerings, ensuring knowledge dissemination to team members.
  • Provide mentorship to paraprofessional staff and foster the company’s mentorship framework within the team.
  • Lead or assist with client satisfaction initiatives, addressing and documenting client concerns and complaints promptly.
  • Demonstrate knowledge of current best practices and standards, liaising with regulatory bodies as needed.
  • Liaise with funders, government programs, case managers, and other interdisciplinary care team members.
  • Participate in recruitment, selection, orientation, training, discipline, and performance reviews of direct reports.
  • Implement, monitor, and evaluate delegated tasks to unregulated care providers.
  • Participate in business development and assist with developing contractual agreements for client care and service.
  • Promote the company through participation in local healthcare or business committees and community events.
  • Advance the effective use of electronic health records and use data to improve clinical outcomes and practice standards.
  • Ensure 24/7 access to a clinician for employees and clients, serving as the on-call nurse/clinician as required.
  • Participate in ongoing professional development and continuing education activities, providing mentorship to other supervisors and administrative team members.
  • Complete other tasks as assigned.
