About The Position

Medica is seeking a Clinical Care Team Supervisor to manage a team of care coordinators providing services for State Public Programs and other Medica members. The role primarily focuses on members within Care Coordination Products (MSHO, MSC+, ISNBC and SNBC) under State Public Programs and the Government division. This position will offer direct supervision to care coordinators and oversight for an assigned panel of members within the Twin Cities Metro and Northern MN areas. The supervisor will partner with the Clinical Care Manager to implement and evaluate care coordination services, identify improvement opportunities, and manage corrective action plans. This role also involves performing other duties as assigned.

Requirements

  • RN Diploma or Degree in Nursing (Associates or Bachelors) + a minimum of 2 years’ experience in community based services, OR Bachelor's degree in Social Work + a minimum of 1-2+ years of community based services
  • 5+ years clinical experience
  • 1-2+ years supervisory experience of professional staff, including employees who are home office or located remotely; informal leadership that has included coaching and mentorship will also be considered
  • Active, unrestricted RN OR SW License in the state of MN Required
  • Eligibility to work in the US
  • Must be legally authorized to work in the United States at the time of application.

Nice To Haves

  • MnCHOICES Certified Assessor Training (MNCAT) preferred or ability to obtain within 21 days of employment
  • Public Health Certification preferred

Responsibilities

  • Managing a team of care coordinators for the provision of care coordination services for State Public Programs and other Medica members.
  • Providing direct supervision to care coordinators.
  • Providing direct oversight for the assigned panel of members.
  • Partnering with the Clinical Care Manager in the implementation and evaluation of Medica Care System care coordination services.
  • Implementing improvement opportunities.
  • Managing corrective action plans as needed.
  • Performing other duties as assigned.
