About The Position

The Supervisor, Care Management provides front line management and support to Clinical Care Managers in the execution of care coordination tasks for members requiring RN and BH care managers. This is a "hands-on" supervisory role, assisting in day-to-day operations and staff supervision while ensuring adherence to departmental expectations. The role involves direct coaching, mentoring, and performance management of the care management team. Responsibilities include recruiting, hiring, onboarding, and ongoing support of the team, ensuring policies and procedures are effectively implemented, and overseeing appropriate system documentation. The supervisor will also serve as a technical and content support for aligning programs and processes with the Strategic Plan, acting as an interdepartmental liaison to monitor service levels and collaborate with internal resources. Additionally, the role involves assisting managers with new projects and processes, providing support to staff with care management tasks (including difficult member situations), and helping staff arrange and coordinate services with community partners. The supervisor will also assist with implementing interventions to improve operational quality and efficiency, perform in-person member visits with staff at least annually, and perform any other job-related duties as requested.

Requirements

  • Associates degree in nursing (RN), Bachelor’s Degree in Social Work, or Master's degree as relevant to licensure eligibility is required
  • Three (3) years of healthcare experience is required
  • Active, unrestricted State Licensed Practical Nurse (LPN), Registered Nurse (RN) license, Social Work license, or Counselor license required
  • Must have valid driver's license, vehicle and verifiable insurance. Employment in this position is conditional pending successful clearance of a driver’s license record check. If the driver’s license record results are unacceptable, the offer will be withdrawn or, if employee has started employment in position, employment in this position will be terminated
  • Influenza vaccination is a requirement of this position. CareSource requires annual proof of Influenza vaccination for designated positions during Influenza season (October 1 – March 31) as a condition of continued employment. Employees hired during Influenza season will have thirty (30) days from their hire date to complete the required vaccination and have record of immunization verified.

Nice To Haves

  • Two (2) years of managed care and/or case management experience is preferred
  • Previous supervisory experience is preferred
  • Case Management certification preferred

Responsibilities

  • Assists the Managers as needed in the review and development of new projects/processes as requested by the health plan.
  • Provide support to staff with care management tasks, including managing difficult member situations.
  • Assist staff in arranging and coordinating services with community partners to maximize utilization of appropriate community resources and support groups.
  • Assist with implementation of interventions to maintain and/or improve the quality and efficiency of the operations/services provided.
  • Perform in-person member visits with staff at least annual and as needed.
  • Perform any other job related duties as requested.
