About The Position

Case Management Supervisor (RN) Baystate Noble Hospital, Westfield, MA Full-Time | 1st Shift (40 hours) Baystate Health, a nationally recognized leader in healthcare quality and safety, is seeking a Registered Nurse Care Management Supervisor to join the team at Baystate Noble Hospital. Position Highlights The Care Management Supervisor leads a team of Case Managers and Utilization Review (UR/UM) Nurses, ensuring high-quality, patient-centered care coordination. This role oversees daily operations, discharge planning, length of stay (LOS), and resource utilization, driving strong clinical and financial outcomes. This role partners with physicians, interdisciplinary teams, and payers to ensure appropriate level of care, medical necessity, and smooth post-acute transitions while maintaining compliance with evidence-based guidelines. Requirements

  • 3+ years of Case Management experience
  • Prior leadership or supervisory experience
  • Active Massachusetts RN license
  • BLS or CPR certification through the American Heart Association
  • Diploma in Nursing
  • Strong knowledge of utilization review and care coordination

Responsibilities

  • Leadership and Team Management Supervise daily workflows, staffing, and performance of Case Management and UR/UM teams Lead hiring, onboarding, training, and staff development Promote a collaborative, patient-focused environment
  • Care Coordination and Utilization Management Oversee discharge planning and transitions of care Ensure appropriate admissions, continued stay reviews, and level of care Monitor length of stay, high-cost cases, and escalations
  • Quality and Outcomes Lead quality improvement initiatives and support evidence-based practice Utilize tools such as InterQual and Midas Partner with physicians and Physician Advisors on complex cases and denials
  • Collaboration and Communication Act as liaison between clinical teams, leadership, and payers Educate providers on length of stay, medical necessity, and payer requirements
  • Discharge Planning Support efficient patient progression and timely discharges Coordinate post-acute care and address barriers to discharge
