Supervisor - Care Management
About The Position
The Supervisor - Care Management coordinates and monitors Care Management team activities and provides leadership, coaching, and mentoring to Care Management and Social Work staff. The role provides leadership and direction for Discharge Planning and Transitions of Care within the acute hospital. It monitors quality indicators to ensure appropriate social and transitional services are provided to patients and families, and develops and maintains relationships with physicians, nursing supervisors, payers, and community resources and agencies to support the needs of indigent, uninsured, and underinsured populations.
Requirements
- Associate’s degree from an accredited school of Nursing, or a master’s degree in Social Work and current Social Work licensure in Georgia.
- Three years of experience in care management, medical social work, or transitional care management.
- Current Georgia Registered Nurse license or NLC/eNLC multistate license, or Licensed Master Social Worker (LMSW) licensure in Georgia.
