About The Position

Under the direction of the Manager, Supervisor, Care Management is responsible for the overall daily operations of the Care Management department for which they oversee. This position will oversee Case Managers and Care Management Coordinator support staff to assure the proper case management of our patients through the continuum of care. The Supervisor, Care Management ensures the Case managers advocate for and assist the patient in the achievement of optimal health, access to care, and appropriately utilizing resources in a manner consistent with the mission vision and core values of Covenant Health. Requirements

  • Graduation from an accredited nursing school, or Bachelor's Degree preferred.
  • Graduation from an accredited social work program, or Master's Degree from an accredited social work program.
  • Upon hire: Texas Registered Nurse License, or
  • Upon hire: Texas Clinical Social Worker License - Texas State Board of Social Worker Examiners, or
  • Upon hire: Texas Masters Social Worker License - Texas State Board of Social Worker Examiners
  • 3 years of experience in an ambulatory and or acute care setting.
  • 2 years of Case Management utilization and/or discharge planning experience in an ambulatory or acute care.

Nice To Haves

  • Bachelor's Degree preferred.
  • Master's Degree preferred.
  • 4 years of experience in an ambulatory and or acute care setting.
  • 3 years of Case Management utilization and/or discharge planning experience in an ambulatory or acute care.
  • 2 years of Lead/supervisory/management experience in an ambulatory or acute care.

Responsibilities

  • Responsible for the overall daily operations of the Care Management department.
  • Oversee Case Managers and Care Management Coordinator support staff.
  • Assure the proper case management of patients through the continuum of care.
  • Ensure Case Managers advocate for and assist patients in achieving optimal health, access to care, and appropriately utilizing resources.
