Supervisor, Care Coordination
About The Position
The Supervisor, Care Coordination assumes responsibility, accountability and leadership for the daily operations including coordination of work, quality, and service. This is a first-line supervisor position in the Department of Managed Care for the assigned site/function. The role facilitates the work of assigned team members and provides a leadership role in ongoing case manager competency assessment, needs identification, and educational offerings. The Supervisor provides educational services and clinical oversight to the Managed Care Coordination team. They also participate in the work activities of assigned teams and provide care coordination services as needed.
Requirements
- Master's Level Degree in Counseling, Social Work or Psychology
- Licensed Clinical Social Worker License
- 3 years Case Management experience
- 5 years Acute Care experience
- 1-year experience working with vulnerable populations
Nice To Haves
- Licensed Marriage and Family Therapist (LMFT)
- Licensed Professional Counselor (LPC)
- Prior supervisory experience
