Requirements

  • Associate's Degree in Nursing (ADN) or Masters in Social Work or equivalent education and experience
  • Minimum of 2 years of clinical care coordination experience, (Utilization Management, Denial Management, Care Coordination) or 2 years of acute nursing team lead, charge or supervisory experience with working knowledge of care coordination
  • Registered Nurse: AZ or Licensed Clinical Social Worker: AZ or Master Social Worker:AZ
  • Certified Case Manager (CCM) , within 24 months

Nice To Haves

  • Bachelor's degree in Nursing, Health Care Administration or related clinical field
  • Understanding of performance improvement activities

Responsibilities

  • Provides direct daily supervision and leadership to the care coordination department, including staff supervision, coaching, counseling and performance management
  • Reinforces care coordination programs and strategies to optimize patient care, resource utilization, and quality outcomes that align with system-wide Care Coordination
  • Ensures adherence to all relevant healthcare regulations, accreditation standards, and organizational policies
  • Assists with the daily operations of care coordination
  • Collaborates with internal and external stakeholders to coordinate patient care and facilitate communication across the care continuum
