About The Position

The Home Care Coordinator ACA Supervisor LPN provides Adult Care Assistant Supervision and ADHC recordkeeping along with some nursing care to participants under the direct and indirect supervision of the Center Manager or designee. The position requires good interpersonal skills and the ability to communicate effectively at all levels. Performs reassessments of Home Care services for program participants, including the coordination of all durable medical equipment, PCA Services and Nursing Facility Care. Responsible for ensuring the safety of the home environment for the participant.

Requirements

• LPN (Licensed Practical Nurse)
• Good interpersonal skills
• Ability to communicate effectively at all levels

Responsibilities

• Provides Adult Care Assistant Supervision
• Performs ADHC recordkeeping
• Provides some nursing care to participants
• Performs reassessments of Home Care services for program participants
