About The Position

At Holon Health, we are rebuilding the healthcare system for justice-involved individuals living with Substance Use Disorder (SUD). We are seeking an experienced and compassionate Nurse Practitioner licensed in North Carolina to support individuals with SUD through collaborative, integrated care. This role involves working closely with a multidisciplinary team and community partners to deliver comprehensive care. The position offers remote flexibility and can be structured as full-time (40 hours/week) or part-time (20 hours/week). Responsibilities include telehealth-based assessments, medication management, and care coordination to support clients on their recovery journey.

Requirements

  • Active and unrestricted NP license in North Carolina
  • Active DEA license (and CDS where applicable)
  • Several years of experience in behavioral health, addiction medicine, or psychiatry
  • Strong documentation skills and proficiency in EHR platforms
  • Clear and confident in virtual communication with both clients and team members

Nice To Haves

  • Multiple and additional state licenses are a plus
  • Board certification in specialty preferred

Responsibilities

  • Conduct health evaluations to assess substance use, co-occurring conditions, and overall functioning
  • Diagnose health conditions and create individualized treatment plans
  • Prescribe and manage medications, monitor for side effects, and adjust treatment as needed
  • Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care
  • Serve as a clinical consultant on complex or high-need cases
  • Educate clients and families on treatment plans, medications, and recovery support strategies
  • Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards
  • Participate in team meetings, professional development, and continuous training
