The Substance Use Disorder Services (SUDS) program provides assistance to teens under probation supervision who may be experimenting with or using alcohol or drugs, but have no previous history or treatment experience. This position works in partnership with the Juvenile Probation Department, with work potentially conducted at Probation offices, assigned courthouses, or in the community. The role involves providing case management for an active caseload of twenty-five to thirty (25-30) youth/wards and interfacing between the Juvenile Probation Department and Community Adolescent Treatment Programs to establish ancillary client services. The SUD Case Manager is responsible for assessment, intervention plan development, and referral services. This includes conducting observed urine sample collections for testing, follow-up, monitoring youth/wards cases within SUDS, and conducting ongoing home, school, and treatment program visits with youth/ward, family, treatment providers, and school teachers. The position requires participation in team meetings, compiling and reporting monthly progress reports to Probation, maintaining case files, entering data into the PCMS database, and providing end-of-month statistics to data entry/admin staff.
