About The Position

The Center for Community Alternatives (CCA) is seeking a Substance Use Counselor to provide counseling services. This role involves identifying, screening, and assessing clients for substance use treatment needs, making referrals to community-based organizations, and coordinating care. The counselor will facilitate therapeutic group and individual sessions and is responsible for completing and maintaining detailed OASAS-compliant case records.

Requirements

  • Must have LMSW/CASAC credentials.
  • Must submit a cover letter with resume.

Responsibilities

  • Conduct intake interviews, biopsychosocials, and mental health screenings.
  • Formulate treatment plans and treatment plan reviews.
  • Act as participant’s case manager to coordinate care within Crossroads and with external providers.
  • Perform individual counseling duties, including holding weekly individual sessions with clients.
  • Provide referrals to community-based resources (healthcare, social services, and administrative e.g. help obtaining insurance).
  • Develop client schedules.
  • Initiate and maintain client records in accordance with policies and procedures.
  • Ensure accurate and timely data entry of all client information into the agency’s database; utilize electronic health record system.
  • Facilitate assigned treatment groups.
  • Perform drug testing as needed.
  • Prepare monthly updates for court or other supervising agencies.
  • Participate in treatment team case reviews.
  • Consult with other professionals in support of client’s treatment.
  • Complete discharge documentation as needed.
