About The Position

Pyramid Healthcare is seeking a compassionate and motivated Substance Abuse Counselor / Co-Occurring Specialist to join our Assertive Community Treatment Team (ACTT) in Winston-Salem, NC. This community-based role focuses on providing direct outpatient Substance Use Disorder (SUD) services to individuals living with serious mental illness and co-occurring substance use disorders. As part of our ACT Team, you’ll deliver recovery-focused care directly in the community — helping individuals access the support and stability they need to thrive. Pyramid Healthcare is a recognized leader in behavioral healthcare across North Carolina, offering individualized, evidence-based treatment services that support long-term recovery and wellness. The Substance Abuse Counselor / Co-Occurring Specialist provides counseling, crisis intervention, treatment planning, and community-based support services to individuals receiving ACTT services. This role works collaboratively with a multidisciplinary treatment team to improve client outcomes and reduce barriers to recovery.

Requirements

  • CADC or LCAS required
  • Ability to obtain required certification/licensure within 90 days of hire (if applicable)
  • Qualified Professional (QP) status per 10A NCAC 27G.0104
  • Valid North Carolina driver’s license
  • Ability and willingness to travel locally throughout the Winston-Salem area

Nice To Haves

  • ACTT, community mental health, or outpatient behavioral health experience
  • Experience working with co-occurring disorders and crisis intervention
  • Strong knowledge of recovery-oriented and person-centered treatment approaches

Responsibilities

  • Provide community-based substance use treatment services in homes and community settings
  • Conduct individual, group, and family therapy sessions
  • Complete LOCAs, pre-authorizations, and clinical reviews in a timely manner
  • Develop and maintain treatment plans, NCTOPPS, discharge plans, and aftercare plans
  • Maintain ongoing communication with families, referral sources, and support systems
  • Deliver interventions for individuals with severe and persistent mental illness and substance use disorders
  • Participate in on-call and emergency service rotation, including crisis response and hospital diversion support
  • Attend treatment team meetings, supervision, and required trainings
  • Complete documentation accurately and within required timelines
  • Perform additional duties as assigned
