About The Position
Crossroads Treatment Centers is seeking a Substance Abuse Counselor to provide individual counseling to highly complex patients. The role involves conducting clinical assessments, designing and updating treatment plans, managing assigned caseloads, and collaborating with nurses and providers for holistic treatment. This position is crucial in combating the opioid epidemic by providing evidence-based medication-assisted treatment (MAT) and helping patients on their path to recovery. Requirements
- General experience working with underserved populations in a clinical setting.
- LMSW
- LCSW
- LPC
- LADAC
Nice To Haves
- Lived addiction experience is a plus.
Responsibilities
- Provides individual counseling to highly complex patients
- Conducts clinical assessments to appropriately assess patient’s treatment needs
- Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning
- Manages assigned caseload and compliance with clinical treatment requirements
- Collaborates with nurses and providers to provide holistic treatment services
- Facilitates urine drug screening for patients
