About The Position
The Substance Abuse Case Manager provides monitoring services for the Health Practitioners’ Monitoring Program (HPMP) for participant adherence to their individualized monitoring contract. The Substance Abuse Case Manager manages all aspects of participant progress through the program including development and implementation of monitoring contracts, ongoing monitoring adherence to contract, and administrative documentation of all aspects of program progress.
Requirements
- Minimum of two (2) years of work experience in chemical dependence treatment and/or mental health treatment
- Bachelors Degree in Counseling, Social Work, Nursing, Psychology or related field from an accredited program
- Licensed Professional Counselor or Certification as a Substance Abuse Counselor preferred
Responsibilities
- Develops and implements monitoring contracts.
- Monitors participant adherence to contracts.
- Provides administrative documentation of all aspects of program progress.
- Supervises the daily assignments of the Case Management Assistants.
