Our mission is to promote health, wellness, and social justice for communities most impacted by HIV through sexual health and substance use services, advocacy, and partnership. We need passionate advocates for social change that embody our five key values: Justice, Dignity, Courage, Leadership, and Excellence. SFAF's Housing & Benefits team provides support to community members seeking rental subsidy and assistance. The Subsidy Coordinator is responsible for working with clients and their service providers to stabilize housing, connect to care, and provide social support. This role will manage a caseload of approximately 75 rental assistance participants and provide each client with comprehensive support and guidance through the program.

Requirements

  • Bachelor’s degree in social work or four years social work, client services or intake experience required.
  • Ability to work with clients with HIV, histories of homelessness, poverty, trauma, incarceration, and/or substance use.
  • Comfort level serving client who are LGBT, varying ethnic origin, varying education level, varying age.
  • Competency in written and oral English language.
  • Ability to maintain files in compliance with HIPPA regulation and professional standards.

Nice To Haves

  • Housing and Financial Benefits experience and HQS Inspection certification preferred

Responsibilities

  • Assist program participant’s housing search and/or retention of current housing.
  • Provide consultation with landlords, property managers, and case conference with medical case managers regarding program policies and troubleshooting client needs.
  • Calculate and document correct monthly rental assistance, generate monthly expense payment requests and draft and communicate changes to assigned subsidy participants and appropriate parties.
  • Provide counseling and practical support in housing, HIV prevention, treatment advocacy (HIV, Medical and Psychiatric Care) and social service support.
  • Provide education and advocacy in regards to Tenant and Landlord rights and responsibilities.
  • Counsel and educate clients on financial benefits to ensure they access relevant local, state, and federal benefits.
  • Facilitate client access to medical care and medications; enroll or refer for ADAP enrollment as necessary, refer to appropriate benefits agencies when indicated.
  • Visit clients in their homes for re-certifications, wellness checks, and quality inspections.
  • Travel to meet with clients in their homes and at area hospitals/care facilities.
