About The Position

Reporting to the Clinical Manager (or designee), the Structural Heart Coordinator is responsible for coordinating all services and systems necessary for a multidisciplinary team approach to the evaluation, treatment, and follow-up of patients requiring structural heart procedures. This includes procedures such as TAVR, Watchman, Mitra Clip, and other emerging procedures. The position reports to the Structural Heart Program Manager and operates under their direction and guidance. Requirements

  • Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
  • Baccalaureate in Nursing with current license to practice as a Registered Nurse in the State of Rhode Island.
  • Certified in Basic Cardiac Life Support (BLS).
  • Certified in Advanced Cardiac Life Support (ACLS).

Nice To Haves

  • Two years related experience as RN in cardiac care preferred.

Responsibilities

  • Is responsible for knowing and acting in accordance with the principles of the Brown University Health Corporate Compliance Program and Code of Conduct.
  • Participates in all visits (consults and follow-ups).
  • In collaboration with MD and APP, conducts patient needs assessments by evaluating and interviewing all new/returning patients.
  • Triages patients to appropriate care resources, including direct contact and telephone encounters.
  • Reviews patient history and other supporting clinical data to facilitate optimum care.
  • Completes nursing assessments, including nursing history, symptom assessment, and medication history and reconciliation.
  • Organizes care conferences with multidisciplinary staff during the screening process and promotes a team approach to the patient program.
  • Effectively explains and educates patients and/or family members about procedures.
  • Teaches based upon assessment, expressed need from patient and/or family, or the initiation of new treatment.
  • Communicates the plan of care and other pertinent information to/from other healthcare providers.
  • Completes patient education.
  • Performs TAVR eval screens, including KCCQ, Frailty, and 5-meter walk test.
  • Serves as a resource to schedulers for questions and problem-solving.
  • Compiles and reviews all diagnostic testing and labs.
  • Coordinates with pre-surgical testing services as needed.
  • Demonstrates a fundamental grounding in nursing theory and practice with mastery of knowledge within a defined content area.
  • Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings.
  • Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model.
  • Assists the healthcare team to achieve mutually agreed-upon healthcare goals by providing necessary support, education, and encouragement.
  • Provides expert consultation for clinical issues, promoting increased knowledge and skills for the consultee.
  • Participates in the education, clinical research, and mentoring of healthcare providers.
  • Facilitates patient/family education in the area of expertise.
  • Annually completes mandatory safety, infection control, and BLS training.
  • Performs other duties as assigned.
