About The Position

Provides coordinated, seamless care for structural heart patients from referral through discharge and follow-up. Works collaboratively with cardiologists, cardiovascular surgeons, nurse practitioners, and the structural heart coordinator. Supports patient assessment, care planning, and education across the full care continuum. Serves as a liaison between patients, families, providers, and referring facilities. Promotes continuity of care, compliance, and positive patient outcomes.

Requirements

Licensed Practical Nurse (LPN) with a current Louisiana license and BLS certification
Minimum of 2 years of experience, preferably in a cardiology or clinic setting
Strong organizational and multitasking abilities in a fast-paced environment
Excellent communication and interpersonal skills with patients, families, and healthcare teams
Compassionate, patient-focused approach with emotional intelligence
Ability to think critically, solve problems, and make sound decisions
Comfortable working independently while collaborating with a multidisciplinary team
Knowledge of healthcare regulations, documentation, and patient care standards
Skilled in patient education and care coordination
Detail-oriented with strong documentation and time management skills

Responsibilities

Assist with pre-, peri-, and post-procedure patient care activities
Coordinate scheduling, testing, and follow-up appointments to ensure efficient care delivery
Act as a primary point of contact for patients, families, and referring providers (including phone triage)
Educate patients and families to ensure understanding of treatment plans and available resources
Track patients throughout the continuum (pre-admission, inpatient stay, 30-day and 1-year follow-ups)
Ensure compliance with Medicare requirements and structural heart registries (e.g., TAVR)
Collaborate with multidisciplinary teams to improve care coordination and patient outcomes
Support data collection, case reviews, and quality improvement initiatives
Build and maintain relationships with referring physicians and external partners
