About The Position

The Structural Heart Navigator supports patients through the care process for valve replacement and/or repair procedures, ensuring a positive patient experience and a smooth transition through the continuum of care. This role serves as a liaison between physicians, nursing, ancillary staff, patients, and families to enhance continuity of care. The Navigator is responsible for coordinating the development of evidence-based patient educational materials and formal patient education opportunities related to Trans-aortic valve Replacement and/or Mitral Valve repair/replacement procedures. They also facilitate the development and implementation of evidence-based clinical practice protocols and orchestrate patient appointments, lab draws, imaging tests, evaluations, and physician consults. Additionally, the role involves collaborating with the Heart team to revise and improve all aspects of the Structural Heart Program, working with unit leaders to implement policies and procedures, and overseeing the maintenance and compliance of Center of Excellence standards. The Navigator identifies staff learning needs and coordinates education for the care of valve patients, assists in resource selection and control, and maintains a Quality Improvement Plan to achieve excellent patient outcomes. This includes coordinating data collection for registries and quality measures, organizing multi-disciplinary teams, attending quality committee meetings, partnering with the Quality Management Department, preparing reports, and participating in planning and organizing special events, open houses, and marketing strategies.

Requirements

  • Bachelor of Science in Nursing
  • 3 years of nursing experience with the Cardiac patient population
  • Good communication skills
  • Good time management and organizational skills

  • Ability to delegate and prioritize care
  • Proficient in Microsoft Office (Word, Excel, Powerpoint)
  • Registered Nurse

  • Ability to flex work schedule to meet patient needs.
  • Ability to travel to various physician offices and community events for purpose of education and oversight of the program.
  • Ability to travel to various rehab centers, extended care facilities, home care agencies and patient homes.
  • Ability to lift up to 35 pounds without assistance.

Responsibilities

  • Serves as a liaison between physicians, nursing and ancillary staff, and patients and families to enhance continuity of care across the continuum of care.
  • Coordinates the development of evidence based patient educational materials and formal patient education opportunities including Trans-aortic valve Replacement and/or Mitral Valve repair/replacement procedures.
  • Facilitates the development and implementation of evidence based clinical practice protocols.
  • Orchestrate patient appointments to clinics, facilitates lab draws, imaging testing, evaluations, and physician consults.
  • Collaborates with the Heart team to revise and improve all aspects of the Program to achieve clinical outcomes and growth targets.
  • Works with unit leaders to implement and maintain established department, unit, hospital policies and procedures.
  • Responsible for the oversight, maintenance, and compliance of Center of Excellence standards for Structural Heart population at Advocate Good Samaritan Hospital.
  • Collaborates with the unit CNS to identify staff learning needs and coordinate evidenced based education for the care of the valve patient population which results in increased knowledge and improved clinical performance.
  • Assists in the selection, provision and control of resources required for performance of duties to include equipment, supplies, space and facilities; establishes standards to ensure cost containment and to optimize working environment.
  • Maintains a Quality Improvement Plan to achieve excellent patient outcomes based on evidence based practices and achieving national benchmarks.
  • Coordinates the collection of data for registries, quality outcomes measures, statistics and follow up of process improvement requirements.
  • Collaborates with unit leaders, physicians, nursing and ancillary staff to improve patient outcomes for safety, infection control, and long term outcomes.
  • Organizes and leads multi-disciplinary teams for promoting and improving services to our valve population patients.
  • Attends quality committee meetings as requested to report on the structural heart program quality indicators and progress.
  • Partners with the Quality Management Department to develop quality indicators used in assessing performance and patient outcomes.
  • Prepares reports and statistics for administrative purposes.
  • In conjunction with unit and hospital leaders, participates/coordinates in the planning and organization of special events, open houses, and marketing strategies.
