About The Position

The Stroke Program Coordinator facilitates the stroke population for the purpose of optimizing the stroke patient's health status, utilizing holistic resources available within the system. The Stroke Coordinator renders highly professional and technical direct or indirect patient care requiring evaluative judgment, specialized knowledge, and professional skills. The individual in this position collaborates with physicians and the multidisciplinary team to ensure the highest standard of care.

Requirements

  • Bachelor's Degree in Nursing
  • Registered Nurse license issued by the state of North Carolina
  • CPR NIHSS
  • Minimum of 5 years of experience in ICU, ED or Neurologic nursing
  • Previous quality improvement experience
  • Assertiveness skills
  • Leadership skills
  • Ability to work cooperatively with a multidisciplinary team
  • Competency in communication
  • Ability to motivate others
  • Exceptional assessment skills
  • Computer skills, including data entry and report generation functions.

Responsibilities

  • Lead the development, implementation, and evaluation of evidence-based stroke care protocols, clinical guidelines, and quality improvement initiatives to ensure optimal patient outcomes.
  • Coordinate comprehensive stroke patient care across the continuum, serving as a liaison among physicians, nursing staff, ancillary departments, patients, and families to promote seamless, patient-centered care.
  • Monitor, collect, analyze, and report stroke program data, quality metrics, and performance outcomes to drive continuous program improvement and regulatory compliance.
  • Develop and deliver education programs for healthcare professionals, patients, families, and the community focused on stroke prevention, recognition, treatment, and recovery.
  • Serve as a clinical resource and consultant for staff, supporting ongoing competency development, evidence-based practice, and interdisciplinary collaboration.
  • Manage stroke patient follow-up, discharge coordination, support groups, and community resources to enhance recovery, education, and long-term outcomes.
  • Maintain expertise in current stroke care standards, treatments, and best practices through continuing education and professional involvement.
  • Support clinical research and grant opportunities by identifying eligible patients, coordinating clinical trial participation, and assisting with program development funding initiatives.
  • Promote quality, cost-effective care through case management, resource utilization, care coordination, and performance excellence across multiple care settings.
  • Uphold exceptional customer service standards while fostering strong relationships with patients, families, physicians, and healthcare team members.
