Stroke Coordinator
About The Position
Responsible for coordinating and guiding the individualized care of trauma/stroke patients through the care continuum, eliminating barriers to healthcare and enhancing patient and physician satisfaction. Serve as a member of the interdisciplinary trauma/stroke team, functioning as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services. Coordinates and works collaboratively to ensure efficient transition of care for trauma/stroke patients. Mosaic Life Care is a health care system in northwest Missouri.
