About The Position

The Street Outreach Worker & Coach is responsible for conducting street outreach to identify, engage, and support homeless and at-risk youth in the community. In addition, the individual will serve as a coach and mentor to Street Outreach Workers employed at sub-recipient organizations, ensuring alignment with program goals, best practices, and the principles of trauma-informed care and positive youth development. This dual role will focus on both direct outreach and capacity building within sub-recipient teams to enhance service delivery across the network.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field. Equivalent work experience may be considered.
  • Minimum of 1 year of experience in street outreach, youth services, or a related field.
  • Strong understanding of trauma-informed care and positive youth development frameworks.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time-management skills.
  • Ability to handle multiple tasks and meet deadlines in a fast-paced environment.
  • Familiarity with local service providers, housing resources, and social service systems.
  • Proficient in Microsoft Office and client management software.

Nice To Haves

  • Experience working with homeless or at-risk youth populations preferred.
  • Prior experience providing mentorship, supervision, or training to a team is highly desirable.

Responsibilities

  • Conduct street outreach to identify and engage youth who are homeless or at risk of homelessness.
  • Build trusting relationships with youth by offering immediate support, resources, and referral to services such as shelter, food, health care, and education.
  • Collaborate with community partners, including schools, healthcare providers, and other service organizations, to locate and assist at-risk youth.
  • Conduct needs assessments and connect youth to relevant services, including housing assistance, counseling, and education.
  • Develop individual case plans with youth, including goals for housing stability and personal development.
  • Maintain accurate and timely records of outreach activities, client interactions, and outcomes.
  • Provide ongoing coaching, mentorship, and technical assistance to Street Outreach Workers at sub-recipient organizations.
  • Conduct regular check-ins with sub-recipient staff to review outreach strategies, challenges, and successes.
  • Facilitate training on trauma-informed care, positive youth development, and harm-reduction strategies to ensure consistency and quality of service delivery.
  • Assist sub-recipient organizations in developing and implementing effective street outreach practices tailored to their community’s needs.
  • Assist in monitoring the performance of sub-recipient Street Outreach Workers to ensure adherence to grant requirements and program objectives.
  • Organize peer learning opportunities and best practice sharing among sub-recipient outreach teams.
  • Provide feedback and recommendations to sub-recipient leadership to enhance service coordination and improve outcomes.
